Bureau of Quality Improvement Services investigations
recording and materials available

As part of the Bureau of Quality Improvement Services’ work to balance facilitating choice, control and community integration with ensuring the health and welfare of individuals receiving Bureau of Developmental Disabilities Services’ services, we use a variety of tools including an incident reporting and management system as well as a complaint system.

Investigations related to these processes are critical in helping providers to gather the factual information of Who, What, Where, When and How. Once this information has been gathered, providers can analyze the incident and identify the Why or the root causes in order to understand how to mitigate future incidents.

On Oct. 26, 2021, BQIS conducted a statewide training on Incident management: Investigations.

Participants learned about:

  • investigation requirements, types and components;
  • what constitutes an effective investigation;
  • and best practices, including root/cause analysis and quality improvement.

A recording of the training is available here: Investigations

Materials offered as resources during the training are now housed on the BQIS webpage under provider resources.

If you have questions, need additional resources or support, or have suggestions for additional training, please contact us at

We look forward to continuing to educate and inspire providers to empower individuals to live, love, learn, work, play and pursue their dreams!
